MacGillan Consulting was founded to answer one question hospitals keep asking after their PET/MRI scanner arrives: How do we create a viable program? We work with hospitals, academic medical centers, pediatric hospitals, and outpatient imaging centers across North America.

WHY MACGILLAN

One modality. End-to-end operational coverage.

PET/MRI sits at the intersection of nuclear medicine, MRI, oncology workflow, and revenue cycle. Each of those domains has its own measurement language and its own consultant pool. MacGillan works across those layers and measures the program as a whole.

Modality-specific.

We work on one modality. Every engagement is informed by direct PET/MRI clinical operations experience, not by a generalist's playbook adapted from CT, MRI or NucMed.

Vendor-neutral.

Our methodology applies across all current-generation PET/MRI platforms in the North American market. We do not accept vendor commissions or referral fees on client purchases.

Operationally current.

The principal consultant maintains active PET/MRI clinical operations alongside this advisory practice. The knowledgebase reflects current operations, not a retired playbook.

Project-management discipline.

Every engagement runs on a PMI-aligned framework: PMBOK® 8th Edition for execution, and CPMAI™ where imaging AI is in scope.

METHODOLOGY

How we approach a PET/MRI program.

A consulting engagement is a project. Treating it like one is the difference between a tidy report and a program planned for the long term.

  1. Frame the problem.

    Stakeholder register, situation statement, scope boundaries, assumptions and constraints. We start with what is true today before we recommend anything.

  2. Measure first.

    Our POMI™ framework (PET/MRI Operations Maturity Index) gives the program a composite number across operational and clinical axes. We do not propose changes until we know where the program sits.

  3. Decide with evidence.

    Financial models cite the CMS rule and year they were built against. Clinical recommendations cite ACR, SNMMI, EANM, or the peer-reviewed evidence behind them. Every claim has a source.

  4. Deliver with stage gates.

    Phase gates at kickoff, mid-engagement, pre-delivery, and close. Each gate runs a domain check and a legal check before the next phase begins. Catching issues in draft is cheap; catching them in a signed deliverable is expensive.

PRINCIPAL CONSULTANT

Derrick Gillan, Founder.

Derrick Gillan founded MacGillan Consulting in 2026 after a decade of hands-on PET/MRI clinical operations across several academic medical centers. He is currently operating a PET/MRI program day-to-day in addition to leading MacGillan engagements.

His direct experience covers PET/MRI program startup, workflow design, protocol strategy, technologist training curriculum development, MRI safety culture programs for staff transitioning from PET/CT or general radiology, cardiac PET/MRI implementation, accreditation preparation, and reimbursement positioning. He has supported the installation and operationalization of PET/MRI systems across multiple institutions and platforms.

He has authored or co-authored peer-reviewed abstracts on PET/MRI clinical applications, including cardiac PET/MRI with investigational tracers. He serves as Vice-Chair of the SNMMI Technologist Section's Emerging Technologies and Radiopharmaceuticals Task Force.

Ryan leads the reimbursement lane on MacGillan engagements: payer positioning, cost-center alignment, and the coverage and coding posture that decide whether a PET/MRI program keeps the margin it earns. He works alongside the principal consultant on Reimbursement Recovery and reimbursement-heavy lifecycle engagements. Scope, confidentiality, and deliverables are contracted through the MacGillan Statement of Work.
